Rodent droppings found in elementary school dish room in Berks County during food safety inspections

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Below are the results of inspections conducted between Oct. 12 and Oct. 25, 2022, that were filed in the state database as of Oct. 27 at 4:30 p.m.

Joe’s Kwik Marts (#1045) Exxon, 2855 Perkiomen Ave., Reading, Oct. 24: six violations. Observed four gallons of milk offered for sale beyond the sell by date of 10/20/22. There were also two half gallons of chocolate milk offered for sale beyond the sell by date of 10/16/2022. Observed two unopened, bulk packs of cheddarwurst sausages in the walk-in cooler with obvious mold on the products. Food facility has sanitizer test strips at the 3-compartment sink, but the container is damaged and the strips are not easily accessible. Corrected. Some soda nozzles and the ice chute at the soda unit, food contact surfaces, were observed to have residue and were not clean to sight and touch. Plastic clam shell containers for the hot dogs are not stored inverted to protect the food contact surface. Food facility has lost its certified food employee over 3 months ago and has not replaced the certified food employee as required. Roof in the storage area behind the front counter is leaking water and is in need of repair. Out of compliance.

Sycamore Valley Farms, 345 Gelsinger Road, Reading, Oct. 24: no violations.

Governor Mifflin School District Intermediate, 600 Governor Drive, Shillington, Oct. 21: no violations.

Mad J's Bbq, 850 Golden Drive Suite 6, Blandon, Oct.21: no violations.

Nuse’s Deli, 3160 Main St., Morgantown, Oct. 21: two violations. Food dispensing utensil in bulk spice containers observed stored in the food and not with handle above the top of the food and the container. Observed a spray bottle of a yellow chemical, located in the men’s restroom, not labeled with the common name of the chemical. Corrected.

Subway #12701, 5479 Pottsville Pike, Leesport, Oct. 21: two violations.Food employee observed in front area making sandwiches not wearing proper hair restraint, such as nets, hats, or beard covers. Corrected. Thermometer for ensuring proper temperature of equipment is not available or readily accessible in the front under-counter refrigerator. Corrected.

Taco Bell 036024, 905 Heritage Drive, Elverson, Oct. 21: two violations. The water temperature of the wash compartment of the manual warewashing equipment was 105°F, rather than 110°F, as required. Corrected. Frozen drink nozzles, a food contact surface, were observed to have black mold like residue and was not clean to sight and touch. The handwash sink located in the dish area does not have water at a temperature of at least 100°F. Corrected.

Wilson School District Spring Ridge Elementary, 1211 Broadcasting Road, Wyomissing, Oct. 21: no violations.

Arbys #7143, 2741 Papermill Road, Wyomissing, Oct. 20: no violations.

Asian Cuisine, 15260 Kutztown Road, Kutztown, Oct. 20: two violations. The handwash sink in the back food preparation area was blocked by several plastic lids and was not accessible at all times for employee use. Corrected. Door located in the back hallway area of the food facility has a gap at the bottom (left side) and does not protect against the entry of pests.

Bethany Children’s Home Main Kitchen, 1863 Bethany Road, Womelsdorf, Oct. 20: four violations. Racks in walk in cooler of main kitchen have slight mold/dirt residue build up on wheels and some wire shelves of the rack. Prep sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Corrected. Food facility is using chlorine Sanitizer at an extremely high concentration of 200 ppms in warewashing machine instead of 50-100 ppms. Caulking at wall at prep table and hand sink area has gaps and must be resealed.

Basket of self-service single-use tableware on counter is not displayed in a manner so that only the handles are touched by occupants.

Corrected. Working containers in seafood counter shelf area, used for storing *chemicals, cleaners* taken from bulk supplies, were not marked with the common name of the chemical. Corrected. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Fan in hallway reach-in refrigerator has a mold/dirt build up also hand sink in kitchen has lime and dirt residue on handles and around sink basin. Protective shielding on lights in open seafood display case is cracked and peeling in front area of 2 cases.

Rabert’s Sunoco & Auto Service, 2403b Route 143, Kempton, Oct. 19: one violation. Food facility is offering for sale foods prepared in an unapproved private home (burritos). Removed from hot hold display.

Wilson School District Whitfield Elementary, 2700 Van Reed Road, West Lawn, Oct. 19: two violations. Mechanical warewashing equipment has an excessive lime scale buildup observed on inside and exterior of machine and drainboards. Faucets at sinks also have lime build up on faucet heads/handles. Majority of dishes in kitchen, metal bowls, trays etc. Have an excessive limescale residue build up on them that is leaving all dishes with white residue.

Brandywine Heights Hs, 103 Old Topton Road, Mertztown, Oct. 18: two violations. Ceiling tiles missing at the wall next to warewashing room and need replaced. Ceiling air exhaust vents of the kitchen/line area are dusty and in need of cleaning.

Frank & Diannah’s Arbor Inn, 47 S. Bingaman St., Reading, Oct. 18: one violation. Walls in the area of the 3-compartment sink and dishwasher are becoming deteriorated. Walls should be non-absorbent and easily cleanable.

Holiday Inn, 6170 Morgantown Road, Morgantown, Oct. 18: four violations. Wall located under and behind 3 bay sink and the walk in cooler, in kitchen is in disrepair and has crevices, rough areas and is not a smooth, easily cleanable surface. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed deep fat fryers, in the cook line area, with encrusted grease and soil accumulation. Observed clean food equipment and/or utensils in ice machine area, stored uncovered or not inverted. Corrected.

Homewood Suites By Hilton, 2801 Papermill Road, Wyomissing, Oct. 18: one violation. Facility does not have a method to verify the final rinse temperature of the hot water sanitizing dishwasher.

Lower Alsace Beneficial Association, 1206 Roosevelt Ave., Reading, Oct. 18: one violation. The handwash sink behind the bar was blocked by a large pot and was not accessible at all times for employee use. Corrected.

Redner’s Warehouse Market #16, 500 Hawkridge Drive, Hamburg, Oct. 18: four violations. Whole cuts of beef were observed to be stored below ground beef products in a sales floor cold holding unit. Corrected. Observed various chip/cookie displays/racks of bread stored directly on the floor in aisle/storage room area, rather than 6 inches off of the floor as required. Corrected. Observed excessive ice accumulation in the walk-in bakery freezer. Working containers in the deli area, used for storing quat sanitizer, taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

St. Catharine of Siena School, 2330 Perkiomen Ave., Reading, Oct. 18: no violations.

Tony’s Italian Restaurant & Pizzeria, 15 N. Home Ave., Topton, Oct. 18: four violations. Observed metal pan rack equipment, in pizza oven area, with an accumulation of dust, dirt on metal wire shelves. Protective taped overwrap on doors of new pizza bain marie must be removed so it is a smooth easily cleanable surface. Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration. Working containers in kitchen area, used for storing *chemicals, cleaners* taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

Ice machine dispenser, a food contact surface, was observed to have an organic matter residue and was not clean to sight and touch. Corrected. Food dispensing utensil in ice observed stored in the ice and not with handle above the top of the ice and the container. Corrected.

Blandon Fire Co #1 Relief Association, Po Box 315, Blandon, Oct. 17: three violations. Interior of ice bin has mold build up along top and sides. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Food debris/grease build up under fryers. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Old strips at bar. Bar sink sanitizer was 100 ppm instead of 200 as required. Corrected.

Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Owner plans take a recertification course in November. No sign or poster posted at the handwash sinks to remind food employees to wash their hands.

Domino’s #4749, 3801 Perkiomen Ave., Reading, Oct. 13: four violations. Floor in the restroom in the back area is cracked / damaged near the toilet and is not a smooth, easily cleanable surface. Floor and part of the wall in the unused restroom is cracked / damaged and is not a smooth, easily cleanable surface. Perimeter walls of the food facility do not protect from entry of insects, rodents, and other pests as evidenced by holes in the walls being plugged with steel wool and other materials. Multiple ceiling tiles are missing in the back area and need to be replaced. There is exposed insulation in this storage area due to the missing ceiling tiles. Food facility inspection still indicates evidence of rodent activity. Facility may need to increase the frequency of their pest control program.

Interior of the ice machine, a food contact surface, was observed to have mold like residue and was not clean to sight and touch. The lid on the outdoor waste receptacle was not closed when not in use. Corrected.

Schuylkill Valley Elementary School, 62 Ashley Way, Leesport, Oct. 12: one violation. Some ceiling tiles missing in the dishwashing room and all the ceiling tiles are missing in the dry storage room. Building is undergoing HVAC work in the ceiling areas. Ceiling tiles need to be replaced as soon as possible or another method needs to be used to prevent any debris from falling into the food areas.

Schuylkill Valley High School, 929 Lake Shore Drive, Leesport, Oct. 12: one violation. Ceiling tile missing in the kitchen area near the office/ovens needs to be replaced.

5th Street Diner, 5340 Allentown Pike, Oct. 15: no violations.

Scrimager Family Meats, 2934 N. Fifth Street Highway, Oct. 15: three violations. Food employee observed in work area, not wearing proper hair restraints, a hair covering or beard cover. This is a repeat violation for four prior year inspections, in 2017, 2018, 2020, and 2021. Food employees shall wear hair restraints such as hats, hair coverings or nets, beard restraints and clothing that covers body hair, that are designed and worn to effectively keep hair from contacting exposed food, clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.
